UMSL Sports Medicine Program embodies a commitment to patient care and education of the student-athlete.
- The health of the student-athlete shall always be our first consideration. By allowing student-athletes timely access to our sports medicine services and by placing a high value on health and wellness, we will reduce the risk of athletic injury while assisting injured student-athletes to a medically safe return to competition in a timely manner.
- We will continually evaluate our Sports Medicine program to ensure our student-athletes have the highest quality sports medicine care.
- We will address problems and concerns regarding the Sports Medicine Department in a timely manner to ensure the needs of our student-athletes and employees are continually met.

UMSL ATHLETIC TRAINING FORMS
All students enrolled at UMSL are required to submit a comprehensive physical form by primary provider upon entry to the school. Please utilize this form to complete the physical - Sports_Physical.pdf. The following documents are in PDF format. Simply click the link or right click and click Save Target As from the pop up menu.
Returning student-athletes who have participated in UMSL Athletics and who already have a physical on file will need a new one prior to their junior year. Although one initial physical is only necessary upon initial enrollment, it is recommended that all student-athletes have an annual complete physical done by their family physician. Urgent care will not be accepted for physicals. UMSL does not perform pre-participation physicals on campus. If a student-athlete arrives on campus without a physical, he/she will have the option to pay a nominal fee to obtain a physical at a local medical facility.
UMSL Sports Medicine Medical Information All freshman and transfer student-athletes must complete these forms annually prior to participating in their first practice at UMSL. The following information must be completed within this packet:

- Student-Athlete Medical History Questionnaire
All student-athletes are required to complete the Sports Medicine Health History form describing previous injuries and past medical history. If a student-athlete has sustained injuries to his/her head, neck, back, shoulder, knee, ankle or internal organs, or has had fractures, dislocations, severe sprains, surgery or any other major health-related problem prior to enrollment at the UMSL, he/she are required to denote on this form. UMSL will not take responsibility for any pre-existing injury which has not been properly rehabilitated or documented.
Previous injuries not fully rehabilitated may disqualify a student-athlete from participation until approval is received from the UMSL Head Team Physician. If a student-athlete is under the care of a physician not associated with the UMSL, written permission from that physician must be received by the Sports Medicine staff indicating when the student-athlete may resume activity and if any limitations exist. However, the final determination regarding athletics participation rests with the Head Team Physician.

Note: Student-athletes under the legal age of 18 years old must have their parent/guardian co-sign all forms prior to participating in athletics at UMSL.

Sickle Cell Trait Test Verification
Per the NCAA rule, we will require all freshmen student-athletes (or transfers) to submit a sickle cell solubility test to be eligible to practice or compete. For more information on sickle cell test and results - here.
Submitting Medical Information
All medical information is transmitted through Healthy Roster. Please note Healthy Roster is an invitation only electronic medical record database that houses injury information and additional medical-related paperwork. New incoming student-athletes will receive a separate email from Healthy Roster to create their account.
